此曼荼羅約製於十二世紀，以白檀木整材雕造。表現修持常行念佛三昧之情景，即修行期間在阿彌陀佛像周圍步行，旋繞經行，口中稱念阿彌陀佛名號，以期見佛。一九三五年指定為日本重要文化財。
圖中大略可分為三層，上層為廡殿頂的宮殿，中央為手結禪定印、綰寶髻戴寶冠的阿彌陀佛，兩旁為脇侍菩薩，均結跏趺坐於蓮座上，前方有十位修行僧念佛右遶經行。宮殿左右有寶樹隔開另造歇山頂式的小宮殿，內有菩薩坐像各一身，其前各配置五身菩薩。
中層有舞蹈的菩薩、人頭鳥身形的迦陵頻伽、供養天人、五身奏樂菩薩及四天王。下層為池中龍頭鷁首的船隻二艘，船上各有一身菩薩，一者合掌，一者搖櫓，兩側為立於岩台上的仁王像。構圖緊湊，人物比例適當，刻工精細，生動逼真，外表施加碎金，呈現淨土之莊嚴。
