內蒙古自治區阿拉善盟額濟納旗黑水城出土。雙頭瑞像事見《大唐西域記》卷二，古印度有二窮人，各發願繪製一佛，但所存積蓄仍不足繪製，畫師被二人誠心感動，便將二人的佛像共繪成一雙頭佛像。二人見畫正生疑慮時，佛像頓現靈異，分身交影，光相滿屋。二人見其瑞相，心生歡喜。
二佛頭左右分開，螺髮高肉髻，面相豐圓，眉間有白毫，雙眉棱起，雙目細長半啟。正二手當胸合十，著袒右袈裟，右肩披覆偏衫，衣紋線條流暢簡單。裙襬以下已失。造型典雅大方，刻劃生動細膩。此瑞像是目前黑水城遺址出土古代雙頭佛像僅有的一件，較為少見。
