菩薩戴高寶冠，寶繒下垂左右對稱，是北魏（386～534）晚期觀音造像衣飾的特徵。面形修長，眉眼細長，面容端莊。身後有同心圓頭光及蓮瓣形火焰紋舉身光。佩項圈，雙肩披帔帛，於腹前交叉繞至身後，下身著長裙，衣褶交疊，飄逸流暢。右手施無畏印，左手結與願印，立於覆蓮台上。台下雙重式四足方台，銘刻發願文「神龜三年（520）三月一日佛弟子韓□□願為亡父見孝內親造觀世音像一區」。
