菩薩高髮髻，戴三葉式寶冠，面形修長，雙眉纖細，鼻大嘴小。身後有蓮瓣形火焰紋背光。身披帔巾與瓔珞，帔帛飄揚外展，瓔珞呈「X」形交叉後下垂至腰際，衣褶簡潔。右手施無畏印，左手自然下垂，立於覆鉢台上。台下為四足方台，方台框右面刻發願文「建義二年（529）己酉七月庚辰十日都□□清造像一軀供奉願育清身見存長□□□□□□正道□願□□□七世先上拾吉發菩提心一切眾生普同」。
