甘肅省酒泉市敦煌市莫高窟第17窟出土。此圖是印刷後再上色，圖上下皆貼藍紙花紋邊飾，與另一張版畫同時被發現，旁邊還放了一串錢幣，似為供養物。
觀音面相豐滿圓潤，頂戴化佛寶冠，胸前佩珠串瓔珞，有頭光及身光，再外為一大月輪。雙目微張俯視，捧持蓮花安詳端坐於蓮座上，蓮座下有海水的波濤。兩旁有一對榜題，分立於蓮花上，楷書「聖觀自在菩薩」、「普施受持供養」。圖下方刊印「聖觀自在菩薩心真言念誦略儀」計十四行。
