根據日本鎌倉弘安五年（1282）台座修復銘文，奈良西大寺的中興祖師叡尊曾延請雕刻師善春、畫師明澄等人補作該像的寶冠、背光及色彩，是日本虛空藏菩薩主尊形式年代最早的遺品，現為日本重要文化財。從其木心乾漆造之技法及作風，推測製作於奈良時代（710～794）末期，顯示出虛空藏信仰之端倪。
菩薩戴五佛寶冠，右手舉起，執嵌有如意寶珠的蓮花，左手置於膝上結與願印，左足垂下，右足盤屈平置蓮座。臉稍長，身軀瘦長，胸前垂飾寶物，帔帛由雙肩垂落腹前，呈「U」形，彎曲的衣紋線條呈現出乾漆特有的厚重感。菩薩身後帶圓形背光，內部有頭光、身光之二重線紋，表面刻鑿紋飾。整體造型洗練簡潔，沉靜莊嚴。
