馬頭明王為觀音菩薩的變化身之一，常作多種造型呈現，如一面二臂、一面四臂、三面四臂、三面六臂以及雙身像等。
此像三面六臂四足，三面均為怒相，戴骷髏冠，紅色毛髮豎起呈火焰形，頂有一馬頭，作嘶鳴狀。身披骷髏鬘，腰繫虎皮裙，六手分別持不同法器，右三手持金剛杵、三股戟、匕首，左三手結期剋印、握短槍和繩索；三股戟、匕首、短槍已佚。四足前後重疊，左展立於覆蓮台上，腳下踩踏七條纏繞的蛇。
