菩薩頭戴三葉花冠，冠旁繒帶向兩側開展呈波浪狀，表現飄動感。臉龐長圓，眉目上揚，面相慈和。雙肩帔帛盤繞卷曲垂至蓮台，戴項圈，瓔珞從雙肩垂掛於腹前交叉穿環而下，富裝飾性。下著長裙，襞褶簡括，邊側線條略見方折。蓮瓣形大背光外層飾火焰紋，內層平面無紋。右手上舉施無畏印，左手下垂結與願印，端立於蓮肉碩大之覆瓣蓮台上，其下為四足方台，方台之足外直內圓。整體造型樸拙，清麗大方，簡潔流暢。
該像四足方台鐫刻銘文「天保五年（554）四月八日燕州廣佞郡下樂縣（今河北涿鹿）人鄭絡興母姊妹二人為父母造觀世音像一區……」。
