該像約雕造於九世紀中葉，現藏於日本京都廣隆寺新靈寶殿。據《廣隆寺資財交替實錄帳》記載，為「安倍朝臣弟澄命婦」之發願造像。此巨大觀音像年代僅次於奈良唐招提寺金堂的古老雕像。一九五三年指定為日本國寶。
觀音軀幹的中心及頭部由檜木整材雕出，背面內刳，嵌上蓋板；部分塗漆、塑形、妝彩，今已全部剝落，呈現古色。頭上包含頂上佛面，共十一面。軀體碩大魁梧，端立於仰覆蓮台上，身後有一環狀頭光。計四十二臂，正面四隻手分別於胸前合掌、腹前捧鉢，其餘從肩後向左右伸展，每隻手皆執器物，其中以錫杖與三叉戟立於正面兩邊，變化中不失勻整。帔帛自左肩斜掛腹前，下裙於腰部返折，襞褶繁複，裙襬呈深刻的漩渦紋樣，刀法勁直，簡潔洗練，線條優美。
