如意輪觀音為密宗六種觀音菩薩造型之一，為祈求長壽、安產和消災免難而廣受崇奉。
此日本如意輪觀音菩薩像呈一面六臂，約雕造於九世紀末至十世紀初。軀體由整材雕成，再接合另刻的手臂與腿部。木刻上留有亮漆、敷彩和貼金裝飾的痕跡。六臂的持物及手印，一般為右邊第一隻手支頤呈思惟狀，第二、三隻手捧寶珠和提念珠，左邊第一隻手放置地上作安掌印，第二、三隻手持蓮花和法輪，但該像的持物已佚。
菩薩姿態優雅自然，整體比例均衡，右腿支起，屈左腿平置蓮座上，作遊戲坐姿。上身披帔帛，下著裙，衣薄貼體，褶紋流暢，技法嫻熟。高聳的頭飾，冥思的神情，豐滿之身形，及深刻的翻波式衣紋，表現當時的雕刻特徵。
