原供奉於日本奈良市唐招提寺講堂，現藏於新寶藏館，為該寺創建時木雕群像之一，約雕造於八世紀中葉，現為日本重要文化財。從頭部至台座軸心由榧木整材雕出。菩薩三目四臂，現左右手各佚一臂。帔帛從左肩斜懸腹前，並於胸腹間打結，具彈性和如獸皮般的質感，此形象與經典儀軌所述的不空●（Q）索觀音共通。
菩薩的鼻端、右耳、胳膊前半部雖已損毀，仍不失其完美形姿。頭綰高髻，束髮盤旋而上，鬢髮垂覆耳前。身軀起伏柔軟，腰身高，腹部內收。上披帔帛，下著裙，衣裙「U」形波紋如細繩狀，棱條凸起，腰帶細刻。造型典雅，技法精湛，結構勻稱，推測係隨鑑真一起渡日的中國唐代（618～907）匠師，或直接受其指導之工匠所製，影響後來日本木雕像甚鉅。
