一九五九年陝西省西安市安國寺遺址出土。此像三面八臂，兩側面大小約為主面的三分之一。主面頭戴化佛高冠，蹙眉瞪目，凝視前方，外露長牙，神情威猛。圓形頭光上方有一馬頭。二主臂當胸合掌，左上手執杵，中手提淨瓶，下手持蓮蕾；右上手執斧，中手持念珠，下手施與願印。身披帔帛，帛帶盤繞雙臂飄垂於蓮座上。瓔珞嚴身，於腹前交叉繫璧垂至蓮座上。身後雕有蓮瓣形火焰紋背光。
整體造型豐腴優美，富有動感，雕刻精細，技法簡練，是唐代（618～907）佛教造像中難得的藝術佳作。
