一九八○年新疆維吾爾自治區和田地區皮山縣杜瓦鎮出土。同時發現的有佛耳、腳、比丘頭像、對鹿範，和半圓柱形疊式花瓣紋、聯珠卷草紋、火焰紋、花卉紋等建築裝飾圖案範。「範」是敬造佛像的模具，這批範用石膏作原料製成，有的底部又用泥土托持以增加牢固度。同時還出土有備製原料的石磨盤工具。
這件佛頭像範額寬，臉呈卵圓形，眉長且細，眉間有白毫，雙目微閉，高鼻嘴小唇厚，下頦有一月牙形凹入。模製出的佛像，臉形圓潤豐腴，面部光滑細膩，表情仁慈安詳。
用模具製造佛教藝術品是社會需求量增加的必然產物；這批皮山出土的佛像模具種類齊全，形象規範，做工精細，手法精練，推測是于闐佛教及其藝術的繁榮時期之作。
