原石存河南省鄭州市登封市少林寺。達摩作梵僧面相狀，頭頂中間高凸無髮，後腦勺周側卷髮，額間有皺紋，眉心糾結，濃眉虯髯，雙目圓睜，高鼻，嘴唇緊閉，戴圓形耳環。身披袈裟，雙手於身前籠袖，衣角隨風飄起，跣足立於蘆葦上。雖無江水之描繪，但人物動態卻明確地呈現渡江的情景。
達摩面容奇古，作者以粗獷的線條勾勒面部及袒露在外的胸部、足部，用筆簡勁流暢；袈裟衣紋的線條則極為厚重樸實，追求線條的厚度與力度以及飛白的效果。兩種不同風格的用線形成鮮明對比，增強了畫面的形式感。
圖左側題詩「不生不滅無今無古空空一／點是為佛祖」，與蕭瑟空寂的畫面意境相吻合。題詩後部及碑左側下部分別有河南太守、登封知縣的款題。
