像作扁平體，耳垂穿孔，原應拴附在板上。菩薩髮髻高聳，戴寶冠，兩側寶繒垂肩。雙眼鑲嵌合金，微鼓。右手持楊柳枝，屈肘上舉平肩，左手下垂執寶瓶，依此造型，應為觀世音菩薩。
菩薩身飾項圈及腕釧，帔帛自雙肩垂於腳踝，自然飄逸。下身著裙，盛飾嚴裝。呈三屈式，立於三足台座。左足已破損。其冠形似西藏喇嘛及蓮花生入藏所戴之僧帽，推測應為七至九世紀西藏作品。蓮花生為西藏紅教之開祖，北印度烏仗那國（今巴基斯坦斯瓦特，Swat）人，足以證明密教於八世紀頃傳入西藏。
