蓮瓣形，邊緣鐫刻聯珠紋。內浮雕九尊坐佛，造型相似，均高肉髻，鼓目，豐唇，長耳及肩，由單層仰蓮座相托；唯印相不同。由上至下，第一層中央主尊作禪定印，兩側有坐佛，雙手合十。第二層主尊右手作觸地印，左手置於腹前；兩側坐佛伸長手臂，手勢未明；主尊左右兩側各塑一立像，四臂，應為菩薩造像。第三層有三尊坐佛，均雙手合掌；主尊兩側各立一塑像，二臂，上身著短外衣。
牌面共有七座佛塔，分別立於頂端和左右兩側。牌下方設有插榫，固定於長方形底座。
