新疆維吾爾自治區吐魯番市勝金口佛寺遺址出土。據《十二天供儀軌》載：「淺青肉色三目忿怒，二牙上出……」此像即是經文中描述的摩醯首羅天。面形長方，額上開天目，粗眉橫鎖，怒目圓瞪，眼珠暴凸斜視，齜牙咧嘴，兩獠牙向上翻翹唇外，滿臉橫肉，眉紅面綠。
