新疆維吾爾自治區和田地區洛浦縣熱瓦克佛寺遺址出土。該遺址四周圍牆內外兩側皆塑有佛像。
該佛頭像原位於西南壁內側，因長年受風吹日曬，且旁邊大佛像倒塌而致該佛像頭部與身軀分離。
佛寬額，面相長圓，五官起伏含蓄微妙，雙眼細長，眼皮以兩泥條貼塑，富裝飾意味，眼球與眉弓交會處勒以淺線，鼻已損毀，唇豐潤柔美、隱約含笑。據斯坦因原考古記載，該像額中原有粉色白毫，現已褪失。
