新疆維吾爾自治區圖木舒克市托庫孜薩來佛寺遺址I殿出土。菩薩臉龐、眉及上、下眼瞼均呈現出優美對稱的弧線。雙目呈杏形，小口緊閉，唇線清晰，嘴形纖巧露笑意，神情溫柔內斂。頭頂束巾綰髻，整體刻劃精巧細膩自然寫實。
此像為該遺址中保存較為完整的菩薩頭像，因在表面上塗薄釉而顯得較為光滑，顯示出匠師在製作過程中的細膩。面部豐腴，五官向內集中，雙目平視，鼻眼小且起伏平緩，為西域地區塑像的特點。
