此造像碑為長形，圖像分三層。上層雕刻太子踰城出家之佛傳故事。圖兩側各有一道蜿蜒的天梯，天梯間有四座代表春夏秋冬四季的宮廷式建築，頂端代表天界之亭閣飾以數層垂帳帷幔。中央太子乘白馬犍陟出城，下方有二天神托蹄，踰城而出。構圖細膩，刻劃鮮明。
碑中層屋形龕內置一佛二菩薩。佛面相清瘦，右手施無畏印，左手作與願印，著寬大袈裟，下裳覆座，衣褶層疊排列整齊。兩側脇侍菩薩帶蓮瓣形頭光，側身而立，身軀微向前傾，姿態輕盈。
下層二亭閣中，各有一胡跪供養人及侍從，底部排列供養人像，雕刻線條簡練概括。
