此碑構圖分為三部分。上部碑首雕雙螭龍，下面方形框內雕九龍浴太子圖。碑身中部開尖拱龕，龕內主尊佛結跏趺坐於台座上，身後有蓮瓣形背光；兩側各雕弟子、菩薩及護法獅。龕上方雕兩排坐佛，龕兩側刻對稱的飛天及力士，最外圍邊框飾卷草紋。碑下部採平面淺雕手法刻供養人、博山形香爐及生活場景，雕工精緻細膩。
碑底部刻發願文「……合邑五十人等信心超俁……上為皇帝陛下為合邑諸人……一時成佛永安二年（529）歲己酉二月……」。據此像風格，應原出河南一帶。
