清宮舊藏。塔高231公分，基座長94公分。塔基為方形束腰須彌座，各級疊澀飾以寶相蓮瓣、纏枝蓮花紋、龜背紋等各色紋飾。橫眉小方框內寶藍色釉地，書有一行金色楷書「大清乾隆甲午年（1774）敬造」。束腰處紫色釉地飾纏枝蓮花紋，每面各有二開光，內飾雲氣紋及二獅相對，開光之間飾十字金剛杵。塔底置紫檀木雕造的蓮瓣紋托泥。
塔基上作三層藍色釉梵文金剛圈，上有覆鉢式塔身。塔身滿飾填彩釉寶相花紋，肩部飾聯珠紋及寶相蓮瓣。正面開龕，內奉三面八臂金菩薩像。
塔身上部以兩層方形剎座承托塔剎。塔剎由十三層相輪、華蓋、仰蓮、日月輪和寶珠組成。十三層相輪象徵佛教的十三重天。
