首為扁圓形，外匝陰刻卷雲紋，內淺浮雕折枝桃，搭配蝙蝠紋飾圖案一組，圖案上下各有一「卍」字。如意柄前窄後寬，略呈波浪形彎曲，中間腹面隆起處陰刻楷書四行律詩一首。柄端浮雕蝙蝠紋飾。
如意係採青白色玉料製作，形似天際流雲，弧度柔和，造型優美，典雅大方。正面柄尾處繫金色長穗。
