甘肅省酒泉市敦煌市莫高窟第17窟出土。繪製於八至九世紀初。悉達多太子出遊四門看見老人、病人、死人、比丘；此圖描繪的是年老及疾病兩個場面。
此圖以邊飾相隔兩個情節，均繪太子騎馬出城，上幅榜題「尒時太子出城東門觀見老人問因緣時」；下幅繪太子看見樹下有一病者，榜題「尒時太子出城南門見一病人問因緣時」。榜題字體工整書寫嚴謹。與同藏於大英博物館，高42.5公分，寬17.5公分的佛傳圖，二者形式相同，尺寸相近，為同一系列畫幡。
