新疆維吾爾自治區阿克蘇地區拜城縣克孜爾石窟出土。作品為奉獻板，上繪佛塔、佛及供養人像。圖上方繪三座塔，中間主塔的塔剎繫二條帶子，與兩旁的小塔相連。主塔下方繪一立佛，色彩已剝落，佛足邊有一跪姿女供養人。立佛下為坐佛，帶多層頭光與背光，結跏趺坐於蓮座上。
圖下半部兩側各有三身坐佛，臉略朝向中間，造型大致相同。圖下方中間繪四身供養人，應是一對夫妻與男童、女童，皆身著西域風格服裝。此木板彩繪顏色多已剝落，曾敷以金箔、銀箔，圖中佛塔的造型可見於克孜爾石窟壁畫。
