此為中國最早的肖像版畫。為中國思州（今貴州思南）華嚴寺僧普南元大德二年（1298）刊本中的木版畫。《圜悟禪師語錄》凡二十卷，係宋代（960～1279）圜悟克勤撰，虎丘紹隆等編。又作《圜悟佛果禪師語錄》、《圜悟語錄》、《佛果圜悟禪師語錄》。編纂於南宋紹興三年（1133），即圜悟克勤禪師圓寂前二年。
此圖中間坐姿僧人為圜悟禪師，雙手執麈尾，倚坐於靠背椅上，榜題「佛果圜悟禪師頂相」。圖左為「虎丘隆禪師」，圖右為「徑山杲禪師」（即大慧宗杲），二人皆為圜悟禪師弟子。
整體構圖主次分明，刀法簡約流利，神情刻劃生動，呈現人物個性特徵。
