甘肅省酒泉市敦煌市莫高窟第17窟出土。依據圖下方中央發願文「清信弟子樊宜信敬造藥師琉璃光如來一鋪」，得知是樊姓像主出資繪製，文末題「天成四年（929）己丑歲五月廿九日題記」，紀年明確。
畫面以一條菱格飾帶區分上下二段，上段主尊藥師佛結跏趺坐於蓮座上，左手托藥鉢，右手持錫杖，其前設香案。兩側為虛空藏菩薩、金剛藏菩薩及二弟子，香案旁的二供養菩薩分別托起花盤及寶盤。
下段願文兩側繪男、女供養人，依榜題知是像主之父母，身後各有一侍者，間繪花樹作為裝飾。
