Avalokitesvara Bodhisattva (back)
韓願造觀音菩薩像

菩薩頭戴高寶冠，冠旁繒帶垂肩，面相豐圓，眉梢、眼瞼上翹，鼻短，五官刻劃率簡，神情端莊肅穆。袒上身，佩項圈，胸前飾帶交叉呈「X」形，腰間繫帶打結，下著長裙，立於覆蓮台上。右手握外展的巾端，左手持蓮莖，肩搭帔帛，繞轉臂間，飄散體側。蓮瓣形背光外層淺刻火焰紋，內層頭光、身光作二重圓環，內飾花紋。背面浮雕觀音菩薩立像。蓮台下為四足方台，正面兩側刻有供養人像，上方飾蔓草紋樣。造型手法疏密有序，具樸拙簡略之特色。
該像四足方台背面銘刻發願文「正始元年（504）十一月三十日高平村韓願為亡妹敬□造觀世音像一區亡者生天見者安隱」，兩側面分別刻「張世□侍佛時」、「清信女□斯侍佛時」。
