甘肅省酒泉市敦煌市莫高窟第17窟出土
甘肅省酒泉市敦煌市莫高窟第17窟出土。圖中十一面觀音幾乎占滿整個畫面。觀音立於蓮台上，主面兩旁為慈悲及忿怒面，寶冠上三層分別有四面、三面及化佛面。觀音主二手於胸前拈蓮花，上二手各托日輪和月輪，下二手分別持淨瓶與念珠。
觀音左腳旁有二身女供養人及一童子，女供養人上方榜題「阿婆壹心供養」，另一女供養人右側題有「阿娘壹心供養」。對面是男供養人，已不易辨識，但仍見題記「燉煌鄉右丞將／隊頭徐奴子壹心供養」，得知供養人為徐奴子。
