甘肅省酒泉市敦煌市莫高窟第17窟出土
甘肅省酒泉市敦煌市莫高窟第17窟出土。繪製於十世紀。保存狀況良好，唯華蓋一角有一片污漬。菩薩有多層頭光，頂上有八面，加上本面及二側面，共十一面。眉間有第三隻眼，身披帔帛，頸上垂下花飾帛帶，斜披絡腋，下著長裙，釧飾、瓔珞嚴身，足踏雙蓮台。胸前二手持蓮花，上二手托日輪、月輪，下二手握細頸瓶及淨瓶。
此像頂上化佛結高髻，並不常見。另外，菩薩二側面斜向前方半側，與一般畫法不同。
