甘肅省酒泉市敦煌市莫高窟第17窟出土
甘肅省酒泉市敦煌市莫高窟第17窟出土。依據作品樣式及下方願文墨書「府主」、「歲次丙辰月日題記」等字樣，推斷繪於五代後周顯德三年（956）。圖左上角榜題為「□□□□空卷索菩薩」，即不空●（上四+下絹）索觀音之聖號，然此像手上卻無●（上四+下絹）索。
觀音立於蓮台上，戴化佛寶冠，黑底白紋鹿皮衣斜搭於左肩。八隻手分持法器或結印相，胸前二手拈蓮枝，上二手握金剛杵，中二手持楊柳枝及結印，下二手提念珠、淨瓶。下端願文兩側各有三位供養人，左為比丘、記有「男憨子」的男供養人和童子；右為男供養人和題為「新婦安氏出適」的女供養人，以及榜題只剩一「女」字的幼童。
