甘肅省酒泉市敦煌市莫高窟第17窟出土。繪製於九世紀下半葉至十世紀上半葉。作品下部佚失，僅存相對二尊菩薩的上半身。
畫面左側菩薩稍側向左，戴化佛寶冠，左手托化佛，右手揚掌。頭光飾尖角形光芒，上方華蓋飾繁花，榜題書寫「南无延壽命菩薩」。
畫面右側為觀音菩薩，稍側向右，左手拈柳枝，右手托琉璃花盞，內插蓮花。頭光飾波形光芒，上方為珠飾帷幔華蓋，頭光旁亦有榜題，未書寫文字。二菩薩相對而立，帔帛翻飛，膚色施以暈染。
