據畫面下方墨書題記，得知為明隆慶六年（1572），即朝鮮宣祖五年，為祈願殿下長壽而繪製。時值文定王后崇佛時期，因而出現不少為祝禱王室之佛畫。朝鮮時期（1392～1910）現存佛畫作品約八十件，麻布設色之作品有三十件。依不同畫布可判別發願者的身分，其中絹本設色者多出自宮廷，麻布設色者多來自民間。
圖中藥師佛結跏趺坐於高台上，戴寶冠，雙手捧藥鉢，身後為二重蓮瓣形紅色頭光和身光。座前左右各立一脇侍，雙手合十，側身相對。周圍有道士圍繞，人物服飾、頭冠各異，臉部表情生動，造型變化豐富。圖最下方之侍者手捧日月，上方虛空布滿雲氣。
構圖對稱，布局緊湊，以朱色為基調，色彩柔和雅致，為十六世紀下半葉朝鮮佛畫之特徵。
