依據《妙法蓮華經》卷七〈觀世音菩薩普門品〉描繪，表現觀音菩薩救度眾生於水、火及其他災難之中。觀音菩薩的信仰，自古以來在印度、中亞、中國、西藏、日本等地均廣為流布。據題款，有經文抄寫者的署名及日期，得知書寫於日本鎌倉正嘉元年（1257），但畫者不詳。
此繪卷高24.1公分，長975.3公分。畫中的情節屬中國樣式，但人物、山水景色及一般細節卻呈現日本本土風味。以賦彩和金泥描繪，筆法細膩，線描輕快活潑，色彩鮮麗，風格寫實，予人清新雅趣之感。
