甘肅省酒泉市敦煌市莫高窟第17窟出土。此圖以菱格飾帶區分上方觀音像和下方的供養人像。觀音髮梳高髻，戴化佛寶冠、耳環、項圈，蓄髭。四臂，胸前兩手結印，上二手托內有金烏的日輪和繪桂樹的月輪。斜披紅色絡腋，下著紅羅裙，結跏趺坐。華蓋裝飾珠串和花朵。
觀音兩側有四位菩薩。上二菩薩呈坐姿，下二菩薩為跪姿，皆有榜題，但難以辨識，現只見靠近觀音頭光左側的「南无虛空藏菩薩」。觀音前方設案。
下方繪六位供養人，左側男子三人，右側二女子及一兒童，中間題記「施主清信弟子常滿兒一心供養」、「□□卓□□一心供養」、「男弟子靈□……」。供養人皆跪於毯上，身邊各有榜題，其中書寫「施主康氏一心供養」及「新婦索氏一心供養」。
