甘肅省酒泉市敦煌市莫高窟第17窟出土。此圖畫面嚴重褪色。菩薩十一面六臂，頭頂有八面，加上主面及兩側的善、惡面，共十一面。主面面容圓潤，五官線條清晰。六臂帔帛纏繞，形成規則的波浪紋。上二手托日輪、月輪，中二手結印，下二手各執淨瓶及柳枝。頂上華蓋飾垂幔、珠寶、吊墜，背景點綴紅色花朵。
下方發願文榜題豎書六行，有五代後周顯德三年（956）紀年。男、女供養人分立願文兩側，各三位，皆有榜題。最左側男供養人榜題「男康住子一心供養」，第二位女供養人榜題「母趙氏一心供養」，其餘榜題無法識讀。
