位於日本奈良縣宇陀市室生區室生寺彌勒堂，為該堂之主尊。推測約雕造於八世紀末至九世紀初期，為該寺創建時期之古像，現為日本重要文化財。
造像從髮髻至台座，包括手臂、帔帛及飾物，由榧木整材雕造，像不刳空，懸垂於胸至足部的瓔珞多為後來補上。頭髮施以群青，朱唇，眉毛、眼瞳及鬚以墨色描繪，外表以素木潤飾完成。
菩薩像高95.4公分，頭部比例較大，膝以下略短，手足稍小，腰身細，具童子身形。左手握長莖蓮花，為後來補作，右手輕拈瓔珞，跣足直立於仰覆蓮台上。頭束高髻，透雕寶冠與頭光為後來補作，刻滿花紋圖案。冠旁繒帶繫結垂掛雙肩，胸前佩戴多重項圈，腹部正中至膝下繫綴長串瓔珞，間飾圓形花紋、方形飾物及珠玉，顯得細緻典雅。衣紋淺刻，身軀柔和，為日本檀像雕刻之佳作。
