該像約雕造於十四世紀，推測原供奉於日本奈良東大寺念佛堂，與閻魔王像同為主尊地藏菩薩像的脇侍。泰山府君又稱為太山王，為冥界十王之一。據傳為司招魂之神，於胎藏界曼荼羅作為閻摩天之眷屬。
該像以檜木多材接合，其中頭部包含衣領在內由一角材，而軀幹則由十塊角材接合，即前後面各四、左右各一等繁複工序而成。眼嵌玉石為後補。頭戴冠，腳穿暖鞋，身著袍服，腰間繫帶，袖口寬大，垂散體側，呈安定坐姿。睜大雙眼，橫豎雙眉，怒目而視，眉間額頭肌肉緊縮鼓起，下頦內收，緊閉嘴唇。右手執筆，手指稍微缺損，筆已佚，左手持二人頭幢，愈顯風骨峭峻，剛正有力。外表仍留存彩色及衣服之紋樣。造型寫實，面貌逼真，加強地獄冥官的威嚴效果。
