河北省保定市曲陽縣修德寺遺址出土。二菩薩姿態左右對稱，各執蓮蕾，立於圓形蓮台上。蓮瓣形背光樸素無任何雕刻裝飾，僅上部高浮雕二飛天，身軀呈「U」形的飛舞姿態，共同托舉一寶塔。方形台座正面中央浮雕博山形香爐，兩旁蹲踞二護法獅，台座兩側為帶有頭光的二力士；博山形香爐與二力士略高於台座。
台座背面銘刻「太寧二年／二月八日珍／妻劉仰為／忘（亡）夫敬造／白玉雙觀／音像一區／普及巳（己）身／無病長受（壽）／所願如是」。北齊太寧年號僅一年，係民間造像者不知其時已為河清元年（562），故謬刻「太寧二年」。北齊（550～577）流行雙尊造像的組合，該像即為其中一例。
