位於開山堂。據《東大寺要錄》記載，奈良寶龜四年（773）良弁示寂，平安寬仁三年（1019）首次舉辦良弁的紀念法會，推測此造像約雕造於當時，為現存最古的良弁肖像雕刻，每年十二月十六日圓寂紀念日開放供人參拜。其一生致力於創建東大寺，受任該寺第一代別當（日本管轄大寺院的僧官）。一九五一年指定為日本國寶。
該像為等身的比丘形象，由檜木整材雕刻，再接合膝部。內著交領僧祇支，外披袈裟，手持如意，結跏趺坐。定睛凝視，表情威嚴，泰然自若，具強韌的意志力，額頭、雙頰刻劃皺紋，被認為是其壯年時期的風貌。背脊伸直，挺胸，肩寬胸厚，雙膝平展，表現碩大骨架之量感。於白底施加膚色、白綠、墨、朱紅等色彩，保存完好，具裝飾效果，展現莊重沉著的性格。
