此像原供奉於日本奈良白毫寺閻魔堂，室町明應六年（1497）因兵火殿堂燒毀，幸存閻魔王像、太山王像、司命像、司錄像四尊。據太山王像胎內銘文記載，鎌倉正元元年（1259）為康圓所作，故推測此閻魔王像約為同時期一系列的作品，現為日本重要文化財。
該像由多材檜木接合而成，眼瞳鑲嵌玉石，外表敷彩。頭戴冠帽，身著道服，右手持笏板，左手置於膝上，半跏趺坐，腳穿鞋，形體魁碩。臉部肌肉緊縮，似凝結成塊，眉毛豎起，怒目瞪視，張口作叱喝狀，表情忿怒。袖袍寬大，衣紋簡略，呈現量感。技法圓熟，寫實逼真，為鎌倉時代（1185～1333）十王像中出色之作。
