此圖舊軸附記，有日本江戶正德四年（1714）裝裱銘文，近年再作修復，煥然一新。繪製於十三世紀。一九○六年指定為日本重要文化財。
圖中以乘六牙白象的普賢菩薩為中心，前方有童子作為前導。帶天竺（今印度）風的象僕，右手執戟，袒上身，跣足立於象旁，另有四尊菩薩伴隨普賢菩薩兩側。
普賢菩薩右足垂踏蓮花，盤坐於象背蓮座上，手持長莖蓮花，蓮花上置三股杵，身著天衣。肩披雲肩，此為婦女披在肩上裝飾物，為中國宋畫常見樣式。四尊菩薩分別手持香爐、孔雀翎毛、中國花卉紋樣和華盤。象身壯碩，鼻銜長莖蓮花，頭戴寶珠，頸項、四足佩有飾物，呈現莊嚴畫面。
此圖以絹背敷彩技法描繪，產生色彩效果，僅從絹表薄施色彩，以保持墨線的筆勢，為鎌倉時代（1185～1333）佛畫的特色之一。
