此圖為三聯式的裝裱屏風，屏風三面高101公分，寬83公分，繪製於十四世紀。圖上二隅書有來迎圖之贊文，其右上開頭記有「弟子天台僧源信」，得知山越阿彌陀圖之圖案設計，為源信之首創。此圖阿彌陀佛的指端仍留有五色彩線，可能係用於臨終儀禮或念佛會時的主尊。一八九七年指定為日本重要文化財。
畫面下方三分之一描繪連綿起伏的山巒，呈現空間的遠近感。主尊阿彌陀佛與觀音、大勢至二脇侍菩薩從山巒上露出半身。阿彌陀佛斂目垂視，手結轉法輪印，頭部放出毫光，採取與觀者正面相對的構圖。圖右側為手捧蓮台的觀音，與雙手合十的大勢至相對，皆面向主尊。三尊身軀為黃色，衣著飾以碎金紋樣，即所謂的全金色像，與暗色背景對比，更顯得輝耀。此圖表現有些形式化，推測是鎌倉時代（1185～1333）後半期之作。
