此圖據《妙法蓮華經》卷七〈普賢菩薩勸發品〉及《佛說觀普賢菩薩行法經》記載，描繪普賢菩薩自東方寶威德上王佛國來此娑婆世界，護持發心受持《法華經》者。此圖菩薩及象周圍的畫絹大部分已失，其餘大致上保存完好。繪製於十二世紀。
圖中菩薩乘六牙白象，合掌趺坐於象背蓮花座上，上方懸垂華蓋，象頭上載有三身化人。菩薩身軀白色，薄施淡朱色暈染，並以淡墨線描繪輪廓。衣服紋樣不使用碎金，而塗以金泥、銀泥表現，並施加細緻色彩，風格優美。象身魁梧健壯，足踏雲彩，雲尾向後拖曳，表現乘雲而來的速度感。象以乘雲姿態出現，不同於豐乘寺本與東京國立博物館本的普賢菩薩像，或許是繪製年代較晚的特徵。
