位於臺灣彰化縣鹿港鎮龍山寺。相傳自明永曆十三年（1659），即由開山和尚肇善禪師自福建溫陵（今泉州）龍山寺迎奉，先供於臺灣彰化縣鹿港鎮的小屋，即最早龍山寺的前身。
此觀音像頭戴毗盧冠，冠前已稍損，頂上結髮髻，髮辮左右繞彎垂至雙肩，五官端正莊嚴，額上白毫。胸飾聯珠紋瓔珞，身著寬袖長袍，褶紋靈活自然，結跏趺坐，雙手結禪定印，懸裳覆蓋方形四足須彌座。
從菩薩的頂上毗盧冠和胸前的瓔珞花紋圖案判斷，應是源於元代（1271～1368）或西藏密教式樣的造型，但衣服的褶紋式樣則接近中原顯教風格，可稱為顯密合流式樣。此像是臺灣毗盧冠觀音像的最早遺例，提供早期密法傳承的線索。
