大仙寺十八臂準提觀音菩薩像

位於臺灣臺南縣白河鎮大仙寺大殿。準提，意譯作清淨，指護持佛法，並為短命眾生延壽護命之菩薩。
準提觀音戴五佛冠，五官端正，遺有明代（1368～1644）風格的沉靜莊嚴，上身帔帛纏結兩臂間，胸前瓔珞嵌綠寶石，腹部有蓮花圖案。左右兩肩向外各有九隻手臂，共計十八臂，其中胸前一對手結印，肩上一對手高舉日輪與月輪，其他各手原有持物，但現或已掉落，或持物過小不易辨認。此像結跏趺坐於仰覆蓮座，蓮瓣寬大，其上刻有「大仙寺佛祖」。
