觀音菩薩臉形稚樸，髮繫帶，帶尾揚起，與背光結合作淺浮雕。右手持長莖蓮苞，左手執衣角，為北魏時期（386～534）觀音常見的造型。披帔帛，自身後繞臂下垂，瓔珞於腹前交叉，下身著裙，陰刻衣褶。蓮瓣形背光內層為倒「U」形，外層飾火焰紋，造型簡單利落。背光背面陰刻一彎樹，樹上一鳥飛過，樹下一半跏思惟菩薩，最下方為六供養人，一男一女成組，造型富拙趣。
尊像、背光與蓮台為一體鑄成。方形四足台座的背面陰刻銘文「太和廿二年（498）十一月一日條□人吳道興為亡父母造光世音一區願居家大家託生西方妙洛國土所求如意兄弟姊妹六人常與佛會」，銘文說明本件為觀音造像，由於翻譯經典的版本而有不同譯名，再加上觀音信仰廣為流行，衍生出不同名稱，如光世音、觀世音等。
