此圖的畫師與發願者不詳，但從其精湛之畫風，推測為十三世紀末至十四世紀初高麗宮廷畫師繪製。其畫面大小僅次於佐賀縣鏡神社的水月觀音圖。現為日本重要文化財。
圖中觀音側身向右，右腿盤屈置於左膝，左足垂踏水中蓮花，半跏坐於岩窟中，身後有竹林及淙淙流泉，為高麗佛畫楊柳觀音像中常見的構圖形式。
圖左上方有一銜著花枝飛來的青鳥，似告知觀音示現之奇瑞。右下方善財童子立於搖曳波間的荷葉上，合掌禮拜觀音。左下方描繪人物捧寶珠禮拜觀音，並有鬼卒隨從背負寶物，甚為罕見。
描線勁健有力，色彩濃淡鮮明，技法表現卓越，優美自然的畫風，被列為絕品之一。
