位於韓國全羅北道南原市山內面立石里實相寺藥水庵。該木刻幀分上下兩部分，以下方的坐佛為中心，周圍有菩薩、弟子立像。一九六五年指定為韓國寶物第421號。
佛結跏趺坐於蓮座上，背光較為厚重；頭部上方有一含苞欲放的蓮花，上有化佛，最高處又刻有碩大蓮花，花中亦有一尊化佛。坐佛左右兩邊各有兩位菩薩，皆立於蓮台上，手上各持有淨瓶、蓮花等。上方中間並列兩位羅漢，左右亦各有兩位菩薩，羅漢與菩薩皆立於蓮台上，手或合十或持淨瓶、蓮花及錫杖等，並以蓮花包藏化佛作為間隔。
木幀兩側飾以向上伸展之蓮花，下端有「乾隆四十七年（1782）壬寅」題記，為研究朝鮮（1392～1910）後期雕刻史提供了重要資料。
