位於講堂。約製作於平安正曆元年（990）講堂重建時期。主尊藥師佛為整木雕刻，脇侍菩薩之軀幹則由左右二材接合而成。一九五二年指定為日本國寶。
藥師佛高247.2公分，結跏趺坐於八重蓮花座上，右手上舉，掌心向前，左手持藥壺，背光外層為透雕卷草紋飾，內層是頭光、身光二重圓相。高肉髻，大螺髮，面相方圓，長眉秀目，額頭正中有一白毫。肩寬胸厚，軀體略呈方形，身披袈裟，衣紋線條簡略、粗大，雙膝褶紋左右對稱，此乃十世紀下半葉的作風。
左右脇侍為日光及月光菩薩，身軀較小，均高172.1公分，具有同樣背光，趺坐於蓮座上。頭戴圓筒形寶冠，身飾臂釧、腕釧，帔帛自右肩斜繞於胸前，衣紋簡潔。日光菩薩左手上揚，拇指、中指相捻，右手置膝上，而月光菩薩之手勢與其相反，作相對姿態，莊嚴肅穆。
